Community Health Assessment & Implementation Plan

In 2016, Sidney Health Center participated in the Community Health Services Development (CHSD) Project administrated by the Montana Office of Rural Health (MORH) and the National Rural Health Resource Center (NRHRC) in Duluth, Minnesota.  A part of this project is community engagement, which includes a health care service survey and focus groups.

The Community Health Services Development project is a process created 20 years ago with the University of Washington. This process helps communities understand what health services they need and to engage the community in strengthening the healthcare system.

The results of this process are available to the public and include two documents:
