Physicians may make the referral for Hospice care, or the patient or family can make a request for Hospice services by calling the office. Hospice will verify if this referral meets criteria through contact with the physician.
Early entry into the Hospice program ensures the opportunity for the patient and family to receive all the benefits of the Hospice program. The physician, Hospice team, patient, and family direct the plan of care; however, the family primarily cares for the patient. Hospice trains the family to participate in patient treatment and encourages open communication.
Security of a caring environment, sustained expert care, and the assurance that the patient and family will not be abandoned, ease the psychological discomfort of the dying process for all involved. Privacy is given to the family in all aspects of care and at the time of death.

| Memorial Service |

A memorial service is held once a year to honor Hospice patients. The Hospice team and families are especially invited to attend this service. |
| Women’s Mend-A-Heart |
March |
An annual one-day retreat for any woman who has suffered a loss. Free of charge. |
| National Hospice Teleconference |
March or April |
The teleconference covers issues associated with grief, to be followed with a local group discussion. Free of charge and open to the public. |
| Camp Mend-A-Heart |
August |
A one-day camp for kids that allows them to deal with losses and feelings. Free of charge. |
| Grief Recovery Support |
Spring & Fall |
Occurs twice a year in the spring and fall, facilitated by a team approach. |
| Musical Concerts & Art Shows |
Annually |
Hospice annually sponsors musical concerts as a fundraiser, as well as an art show for bereavement healing. |
| Parade of Lights |
December |
Hospice participates in the Sidney Chamber Christmas Parade of Lights and the MonDak Heritage Center tree decoration each year to celebrate life and honor our lost loved ones. |
