Home & Community Based Services began out of a growing need for families to have support in the home while caring for a family member, as well as for individuals who want to remain independent in their home but need some assistance.
Home & Community Services staff consists of a Registered Nurse and a Medical Social Worker whose job is to assist families in arranging care to meet their needs.
The Case Management Team provides assessment, planning, arranging, implementing, and monitoring of any necessary services for an individual. A person must be Medicaid eligible and need assistance in their home in order to be eligible for this program.
